Critical Illness Insurance

Critical Illness Insurance - Policy pays a lump sum benefit if the insured is diagnosed with a covered critical illness in each of two categories (three categories with the Cancer Rider). Category One includes benefits in the event of a heart attack, stroke, heart transplant, by-pass surgery, and others. Category Two includes benefits in the event of major organ transplant other than heart, end stage renal failure, multiple sclerosis, Alzheimer’s disease, and paralysis not as a result of stroke. If selected, the Critical Illness Cancer Rider pays a lump sum benefit in the event of a covered cancer diagnosis. Policy continues in force until full benefits are paid in all categories, as long as premium payments continue. A Wellness Benefit Rider may be added, offering an annual benefit if the insured has a covered screening test.



Universal Life Insurance

Universal Life Insurance offers fund value accumulation, a competitive interest rate and can help ease the financial burden brought upon by the death of a family’s primary provider.

Universal Life Insurance – Two policies are offered through payroll deduction to meet the needs of the insured with premiums from $5.00 a week. UL20P and UL21P both have minimum issues in most states of $10,000 and are issued on a unisex basis. UL20 and UL21 offer competitive interest rate. Tax-deferred accumulation of fund value and potential for loans and partial withdrawal based on fund value are just a few highlights of the policies.



Term Life Insurance
Life Insurance offers an economical method of owning life insurance to cover a specific need for a specific amount of time.

Term Life Insurance – Offers an economical method of owning life insurance to cover a specific need for a specific amount of time. Horizon 20 year insurance is available through payroll deduction or direct billing with a $10,000 minimum issue limit. Current premiums guaranteed for 5 years.



Cancer and Specified Disease Insurance

Policies for Cancer only or Cancer and Specified Disease Insurance. Pays benefits, in addition to existing coverage, that can be used to help cover the costs associated with the treatment of cancer and other specified diseases.

Cancer and Specified Disease Insurance – Policy benefits can be used to pay the difference between what primary medical coverage pays and the total costs (medical and non-medical) of care. Benefits are paid directly to the insured unless assigned, and coverage is available for individuals and families in the payroll market.

The Heritage Provider – Provide benefits that correspond with cancer treatment, including: bone marrow transplants, prosthesis and reconstructive breast surgery, comfort and anti-nausea medicine, blood, plasma, and platelets, and continuous hospital confinement.



Accident Insurance (with Expanded Benefits)
Accident Insurance provides benefits for on- and off-the-job accidents or off-the-job only accidents and pay a lump sum benefit to the insured that can be used to help cover the costs not paid by the primary insurer. Riders can be added to expand benefits. Read more

Accident Insurance (with Expanded Benefits) – Choice of a policy with off-the-job only or on- and off-the-job coverage. The policy pays benefits that can be used to help cover costs that current insurance may not cover. Customizable coverage to fit the insured’s budget with the Silver, Gold, and Platinum or Basic, Enhanced and Premier coverage options with riders to enhance coverage. Benefits offered by the policy correspond with: loss of life or limb, dislocation or fracture, hospital confinement, ambulance, medical expenses, and disability (primary insured only).



The Benefit Enhancement Rider is packaged with the base policy in the Silver, Gold and Platinum packages and pays benefits for the following: accidental follow up treatment, physical therapy, lacerations, tendon, ligament, rotator cuff, or knee cartilage surgery, ruptured disc, appliance, non-local transportation, family member lodging, brain injury diagnosis, eye surgery, burns, skin graft, initial hospital confinement, blood and plasma, hospital intensive care unit confinement, open abdominal or thoracic surgery, prosthesis, paralysis, and coma with respiratory assistance. Hospital Indemnity Insurance (Supplemental Health Options Plan)

Hospital Indemnity Insurance referred to as SHOP allows the insured to "shop" for the supplemental health benefits that best meet their budget and needs.

Hospital Indemnity Insurance – Referred to as SHOP (Supplemental Health Options Plan) because it allows the insured to “shop” for the benefits that best meet their budget and needs. Silver, Gold, and Platinum packages are available to meet many needs with optional riders added to the policy to broaden its coverage. Benefits are paid under the Base policy for daily hospital confinement, hospital intensive care unit and waiver of premium. SHOP is available for individuals and family.



Heart & Stroke Insurance

Heart & Stroke Insurance pays benefits, in addition to existing coverage, that can be used to help cover costs associated with the treatment for heart & stroke related conditions.

Heart & Stroke Insurance – Pays benefits that can be used to cover expenses health insurance may not cover. The benefits are paid as the insured goes and corresponds with specific treatments and expenses (up to the maximum allowed). Coverage also pays in addition to other policies the insured might have such as medical or disability. Some of the benefits include: Hospital Confinement Physician’s Attendance, Inpatient Drugs and Medicine, Heart Transplant and Coronary Angioplasty. The Heart & Stroke policy helps the insured manage the high expenses of treatment to preserve their savings and protect them from financial hardship, allowing them to focus on getting better.



Disability Income Insurance

Disability Income Insurance provides an income when the insured can’t work because of a covered sickness or off-the-job accident.

Disability Income Insurance – Provides an income when the insured can’t work due to a covered sickness or off-the-job injury. Benefits are provided in $100 units from $400 to $5,000 or up to a percent of income. Affordable rates are age banded at the time of issue and the policy is portable, meaning if the insured changes jobs, he or she has the option to continue coverage as long as premiums are paid to the insurance company.
