Allstate Workplace Division's group voluntary products have minimum participation requirements and may vary by product.
Group Term Life InsuranceTerm Life Insurance coverage can ease the financial burden brought upon by the death of a family’s primary provider. Accidental Death and Dismemberment can be added and provides benefits in the event of death or loss of limb resulting directly or independently of all other causes from accidental bodily injury.
Provides Term Life Insurance for the employee and their dependents. Coverage is in addition to any coverage already provided by the employer.
Group Universal Life Insurance
Universal Life Insurance coverage provides a death benefit and over time, provides tax-deferred fund value accumulation that may be used for retirement, children's education, or emergencies.
Contingent Guarantee Issue Group Universal Life Insurance Coverage is available to employees of qualified groups. Several Riders are also available. This is in addition to any coverage already provided by the employer.
Short Term Disability
Short Term Disability coverage provides monthly benefits for a period of up to 26 weeks.
Provides a source of income if the insured can’t work because of a covered sickness or off-the-job injury. Benefit amounts can be chosen in $100 increments from a weekly benefit of $400 up to $2500 or 60% of earnings, which ever is less. Payment periods are offered for 3, 6, 12, and 24 months at a 1 year guaranteed rate.
Long Term Disability
Long Term Disability coverage provides monthly benefits for an extended period of time.
Provides a long term source of income if the insured can’t work due to a covered sickness or injury. Benefit amounts can be chosen in $100 units from $400 to $6000 or 60% of earnings, whichever is less. Rates are age banded and are the same for both males and females. Maternity is covered the same as any other illness and the plan also offers Partial Disability benefits, Waiver of Premium and Recurrent Disability benefits.
Dental
Dental coverage provides dental coverage with a built in wellness benefit.
Percentage of coverage increases during the 2nd and 3rd years of the policy. After six months of coverage, Orthodontic Services and Braces are also covered for dependent children under the age of 19.
Limited Benefit Medical Plan
AHL minimedical® coverage offers a scaled down health plan with annual limits to keep the plan affordable.
Referred to as AHL minimedical®, this plan is designed for part-time and entry level employees. Benefits include Medical Expense Insurance, PPO savings, wellness benefits, hospital indemnity coverage, prescription drug benefits, and accidental death and dismemberment coverage. Voluntary dental, term life, short term disability, accident and hospital indemnity benefits are also available to personalize coverage.
Critical Illness
Critical Illness coverage pays lump sum benefits directly to the insured at the time a covered illness is diagnosed.
Critical Illness coverage pays lump sum benefits directly to the insured at the time of diagnosis for heart attack, stroke, heart transplant and other threatening conditions as defined in the certificate. Options are available for cancer, wellness, and recurrence benefits.
Cancer and Specified Disease
Cancer and Specified Disease pays benefits that can be used for non-medical related expenses that health insurance may not cover as a result of cancer and 29 other specified diseases.
Pays benefits that can be used for non-medical cancer related expenses that health insurance might not cover. Benefits are paid as they go and cover the cost of specific treatments and expenses as they happen. Covers 29 other diseases and offers options.
Accident
Accident coverage pays lump sum benefits for on- and off-the-job accidents, in addition to existing medical coverage.
Pays lump sum benefits for on- and off-the-job accidents. This plan provides 24 hour accident benefits for dislocations, fractures, loss of limbs, and death as a result of an accident. Hospital, Intensive Care and other medical expenses are covered in the Accident plan.
Hospital Indemnity
Referred to as Group SHOP, (Supplemental Health Options Policy), pays predetermined amounts for covered hospital, outpatient, nursing, transportation and physician services. Optional benefits include prescription drug coverage; wellness and preventative testing; diagnostic X-rays; off-the-job accident and sickness disability coverage; and term life coverage.
Group Indemnity Medical
A limited benefit supplemental hospital indemnity policy available to employees of qualified groups.
A Group Limited Benefit Plan with Indemnity Schedule Benefits combined with PPO access and savings for patients. Choice of pre-packaged plans. Coverage available for employee or employee and family.
Group Vision
A Group Limited Benefit Plan that provides benefits for vision care, with an affordable co-pay schedule for Preferred Providers and one for Providers of your choice that are not on the Preferred list. Benefits for Vision Examinations, Lenses and Frames, and Contact Lenses. Coverage available for employee or employee and family.
